This newest technique has enabled endoscopists to assess the microstructure of gastrointestinal mucosa <italic>in vivo</italic> during the endoscopic procedure ("virtual biopsy"), and has stimulated growing interest to its application in clinical practice. During colonoscopy, the use of confocal laser endomicroscopy is justified for assessment of post-resection margins after excision of epithelial neoplasms, for differential diagnosis of polyps and assessment of the colon in patients with inflammatory bowel disorders. This article presents the interpretation criteria of confocal images in various colon diseases and the published data on the diagnostic value of the method.</p></abstract><trans-abstract xml:lang="ru"><p>Конфокальная лазерная эндомикроскопия – быстро развивающееся направление в гастроэнтерологии, представляющее собой мост между эндоскопией и гистологическим исследованием. Эта новейшая  технология  обеспечила  возможность изучения микроструктуры слизистой оболочки  желудочно-кишечного  тракта  <italic>in vivo</italic> во время  проведения эндоскопического  исследования («виртуальная биопсия») и вызывает все больший интерес к применению в клинической практике. При исследовании  толстой кишки применение конфокальной эндомикроскопии оправдано для  оценки  краев  резекции  после удаления эпителиальных образований, для дифференциальной диагностики полипов и оценки состояния толстой кишки у больных с воспалительными  заболеваниями кишечника.
