From these, 87 patients had CKD stage I, 29 with CKD stage II and 40 with CKD stages III, IV, V. For the first time in Russia, the authors summarize their unique experience in management of pregnancy with CKD, underline a high probability (27,5%) of its primary detection during pregnancy, discuss the algorithms of assessment, prevention and treatment of various gestational complications in CKD (pre-eclampsia, urinary tract infections, feto-placental insufficiency, anemia, acute renal damage), as well as the influence of pregnancy on renal function at long-term post-delivery. A direct correlation between the CKD stage, frequency of pre-eclampsia, feto-placental insufficiency, preterm deliveries, surgical deliveries by caesarean section and babies’ status at birth is demonstrated.</p><p>Based on their ample clinical material, they confirmprobability of favorable pregnancy outcomes in CKD patients with stable renal function without severe arterial hypertension during pregnancy: for a baby in 87%, for the mother in 90% (maintenance of the same CKD stage). The risk of persistent deterioration of renal function during pregnancy and puerperium in women with CKD is higher in CKD stage IV, as well as in the case of early development of pre-eclampsia; it also correlates with severity of the latter.</p><p>The probability of a favorable obstetric and nephrological outcome is higher when the pregnancy is planned and intensively co-managed by an obstetrician/gynaecologist and a nephrologist from early weeks of gestation onwards.</p></abstract><trans-abstract xml:lang="ru"><p>Беременность у женщин с заболеваниями почек даже при сохранной почечной функции сопровождается повышенной по сравнению с популяционными показателями частотой акушерских и перинатальных осложнений, таких как присоединение преэклампсии, преждевременные роды, необходимость оперативного родоразрешения, проведения интенсивной терапии новорожденным. </p><p>В статье приведены собственные данные об осложнениях и исходах беременности у 156 женщин с различными стадиями хронической болезни почек (ХБП). Из них 87 пациенток были с ХБП I стадии, 29 – с ХБП II стадии и 40 с ХБП III, IV, V стадий, объединенных в диагноз «хроническая почечная недостаточность» (ХПН). Впервые в России авторами обобщен уникальный опыт ведения беременности при ХПН, подчеркнута высокая вероятность (27,5%) ее первичной диагностики во время беременности, представлены алгоритмы обследования, профилактики и лечения различных гестационных осложнений при ХПН (преэклампсия, инфекции мочевыводящих путей, фетоплацентарная недостаточность, анемия, острое почечное повреждение), а также влияние беременности на функцию почек в отдаленном послеродовом периоде. Доказана прямая корреляционная зависимость между стадией ХБП, частотой присоединения преэклампсии, фетоплацентарной недостаточности, преждевременных родов, оперативного родоразрешения путем кесарева сечения, состоянием детей при рождении.</p><p>На основании большого клинического материала доказана вероятность благоприятного исхода беременности у пациенток с ХПН при стабильной функции почек и в отсутствие тяжелой артериальной гипертонии во время беременности: для ребенка – в 87%, для матери – в 90% (сохранение той же стадии ХБП). Риск стойкого снижения функции почек во время беременности и в послеродовом периоде у женщин с ХПН повышается при ХБП IV стадии и в случае раннего присоединения преэклампсии, а также коррелирует с ее тяжестью.</p><p>Вероятность благоприятного акушерского и «нефрологического» исхода повышается при планировании беременности и интенсивном совместном ведении пациенток акушером-гинекологом и нефрологом с ранних сроков беременности.</p></trans-abstract><kwd-group xml:lang="en"><kwd>chronic kidney disease</kwd><kwd>chronic renal failure</kwd><kwd>pregnancy</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>хроническая болезнь почек</kwd><kwd>хроническая почечная недостаточность</kwd><kwd>беременность</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>1.
