The study included 4422 adults (≥ 18 years) with uncontrolled type 2 diabetes (HbA1c &gt; 7 and ≤ 11%) with ≥ 1 oral anti-hyperglycemic drug and for whom the treating physician had decided to add Gla-300. We performed a post-hoc sub-analysis of the study participants recruited in Russia.</p> <p><bold>Results</bold>: The Russian study group included 1493 patients receiving Gla-300. At 6 months, 25.9% of the patients achieved their predefined individualized HbA1c target and 53.3% achieved their HbA1c target at month 12. Their mean (± SD) HbA1c level decreased from 9.3 ± 0.9% at baseline to 7.6 ± 0.7% and 7.2 ± 0.7 at months 6 and 12. The incidence of hypoglycemia was generally low; overall, severe hypoglycemia was reported only in 0.07% and 0.13% of the patients at 6 and 12 months. The baseline average daily dose of Gla-300 was 13.2 ± 4.9 Units; it to 23.6 ± 9.1 and 26.0 ± 9.8 Units at months 6 and 12.</p> <p><bold>Conclusion</bold>: In the real world setting, initiation of insulin Gla-300 in type 2 diabetic patients who had been out of their target glucose range with oral hypoglycemic agents is associated with improved glycemic control and low risk of hypoglycemia.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Обоснование</bold>. Базальный инсулин гларгин 300 ЕД/мл (Гла-300) – аналог инсулина второго поколения – характеризуется сопоставимой эффективностью и меньшей вариабельностью в сравнении с аналогом инсулина длительного действия первого поколения гларгином 100 ЕД/мл.</p> <p><bold>Цель</bold> – оценить эффективность и безопасность применения инсулина Гла-300 у пациентов с сахарным диабетом 2-го типа (СД2), ранее не получавших инсулинотерапию, в реальной клинической практике в России.</p> <p><bold>Материал и методы</bold>. ATOS (NCT03703869) – проспективное наблюдательное международное многоцентровое исследование продолжительностью 12 месяцев. В исследование включены 4422 пациента старше 18 лет с СД2, не достигшие целей гликемического контроля (уровень гликированного гемоглобина (HbA1c) &gt; 7 и ≤ 11%) на 1 и более пероральных сахароснижающих препаратах, которым лечащий врач решил добавить к терапии Гла-300. Проведен незапланированный протоколом дополнительный анализ популяции пациентов из России, принявших участие в исследовании.</p> <p><bold>Результаты</bold>. Российская популяция исследования ATOS составила 1493 пациента, получавших инсулинотерапию Гла-300. За 6 месяцев терапии индивидуального целевого уровня HbA1c достигли 25,9% пациентов; за 12 месяцев терапии – 53,3%. Средний уровень HbA1c снизился с 9,3 ± 0,9% исходно до 7,6 ± 0,7% через 6 месяцев и до 7,2 ± 0,7% через 12 месяцев. Частота гипогликемии в целом была низкой; в течение 6 месяцев тяжелая гипогликемия возникла только у 0,07% пациентов и в течение 12 месяцев – у 0,13%. Исходная средняя суточная доза инсулина Гла-300 составляла 13,2 ± 4,9 ЕД в сутки; к 6-му месяцу доза возросла до 23,6 ± 9,1 и к 12-му месяцу – до 26,0 ± 9,8 ЕД в сутки.</p> <p><bold>Заключение</bold>.
