Assessment of hemodynamic parameters and blood gases was done at admittance to the intensive care unit (ICU), as well as at 6 and 24 hours after surgery. Lung ultrasonography was also performed including counting of B-lines in 12 lung quadrants at 6 and 24 hours after surgery, as well as chest radiography at 24 hours. Duration of mechanical ventilation, time in ICU and in-hospital stay were also evaluated. <bold>Results</bold>: Gas exchange deterioration was associated with increased numbers of B-lines: 9 (5 to 15) at 24 hours after surgery. In the patients with PaO&lt;sub&gt;2&lt;/sub&gt;/FiO&lt;sub&gt;2&lt;/sub&gt; above 300 mm Hg the number of B-lines at 24 hours after surgery was 4 (2 to 8) (р = 0.04). Plain chest radiography at 24 hours after surgery revealed abnormalities in 69% of the patients. Discoid atelectases were the most common findings (n = 13). The ROC analysis showed that increased numbers of B-lines above 10 at 6 hours after completion of the surgery was predictive of the development of X-ray abnormalities at 24 hours (AUC 0.82, р = 0.02, sensitivity 86%, specificity 76%). At 6 hours after the intervention the patients who subsequently required prolonged mechanical ventilation had increased numbers of B-lines (15 [14–27]) compared to those who could be extubated within the first 24 hours after surgery (10 [3–13], p = 0.02). <bold>Conclusion</bold>: Lung ultrasonography monitoring accelerates the diagnosis of respiratory problems after cardiac surgery and allows timely identification of the patients requiring prolonged respiratory support and ICU stay.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Актуальность</bold>. Наряду с обзорной прикроватной рентгенографией органов грудной клетки для выявления послеоперационных дыхательных осложнений все чаще используется ультразвуковой контроль состояния легких. Целью нашего исследования была оценка эффективности ультразвукового контроля легких в выявлении послеоперационных дыхательных осложнений у пациентов после кардиохирургических вмешательств, выполненных в условиях искусственного кровообращения. <bold>Материал и методы</bold>. В исследование было включено 39 пациентов, перенесших плановые кардиохирургические вмешательства в условиях искусственного кровообращения. При поступлении в отделение интенсивной терапии (ОИТ), а также через 6 и 24 часа после оперативного вмешательства всем пациентам выполнялась оценка показателей гемодинамики и газообмена. Кроме того, проводили ультразвуковой контроль, включающий подсчет количества В-линий в 12 квадрантах легких через 6 и 24 часа после операции, а также рентгенологическое исследование органов грудной клетки через 24 часа после вмешательства. Учитывали длительность искусственной вентиляции легких, время пребывания в ОИТ и стационаре. <bold>Результаты</bold>. Ухудшение газообмена было ассоциировано с большим числом B-линий: 9 (5–15) спустя 24 часа после оперативного вмешательства; при этом у пациентов с PaO<sub>2</sub>FiO<sub>2</sub> более 300 мм рт. ст. количество B-линий через 24 часа после операции составило 4 (2–8) (р = 0,04). Изменения на обзорной рентгенограмме органов грудной клетки были зарегистрированы через 24 часа после оперативного вмешательства у 69% пациентов. Наиболее часто отмечали наличие дисковидных ателектазов (n = 13). Проведение ROC-анализа выявило, что увеличение количества В-линий более 10 спустя 6 часов после завершения оперативного вмешательства позволяло предсказать развитие рентгенологических изменений через 24 часа после операции (AUC 0,82, р = 0,02, чувствительность 86%, специфичность 76%). Через 6 часов после вмешательства количество В-линий при ультразвуковом исследовании было больше у пациентов, которым в дальнейшем потребовалось проведение продленной вентиляции легких: 15 (14–27) по сравнению с 10 (3–13) у больных с экстубацией трахеи в течение первых 24 часов после операции (p = 0,02). <bold>Заключение</bold>.
